The Leesburg Police Department is working in cooperation with the Lady Lake Branch of the Alzheimer's Resource Center's Support Group, the Alzheimer/Dementia Awareness Association (A.D.A.A.), and other volunteer support groups, in addition to local, state and federal agencies in the establishment of an Alzheimer's Wanderers Registry.

How The Registry Works

The Leesburg Police Department in cooperation with volunteers, will be acting as the registering agency. To register an "Extra Special Person", simply contact one of the volunteers or contact Sr Ofc Marlene Frost at the Leesburg Police Department by calling 352-728-8211 or by email. You will be asked to sign a waiver allowing the department to use this information to return the Alzheimer/Dementia victim to you or their other care givers. This will also allow the department to release information concerning the victim to other emergency agencies as necessary. Information will not be released to anyone other than authorized emergency agencies such as fire/rescue, EMS, and hospitals. Information will also be released on a "need to know" basis only.

The enrollment form contains physical characteristics of the Alzheimer/Dementia victim, medical data, and a point of contact in case of emergency. A copy of the form will be retained by the registering agency. The original form and photograph will be forwarded to the Leesburg Police Department where it will be entered into the computer system and available to police officers 24 hours a day.
